Adapting to an Evolving Healthcare Landscape

In an era defined by technological advancements and innovative solutions, healthcare is no exception. One significant development is the rise of home hospital care programs, offering a novel approach to patient care delivery. Dr. Amanda Lucier, a fourth-year resident in the Department of Medicine, reflects on her experience with the @MassGenBrigham Home Hospital program. This initiative aims to deliver hospital-level care to patients in their homes, redefining traditional models of care.

A New Approach to Patient Care

Massachusetts General Brigham, in collaboration with Beth Israel Lahey Health, has introduced home hospital care programs at three local hospitals on Cape Ann and the North Shore. This initiative brings a home-based alternative to facility-based stays, allowing patients to maximize care and recovery time within the comfort of their own homes. Not only do patients have 24/7 access to their care teams, but typical services like lab tests, IV therapies and medications, oxygen, and radiology studies are all available directly in the home.

Leading the Way in Home-Based Hospital Care

Mass General Brigham has been a key player in the hospital-at-home space, launching its program in 2016. This initiative predates the surge in similar programs following the CMS Acute Hospital Care at Home waiver in 2020. Despite financial losses for the fiscal year ending September 2023, due to ongoing capacity and labor challenges, the system's focus remains on delivering excellent patient care, sustaining research, educating healthcare providers, and serving the community.

Success Across the Nation

Across the U.S., hospital-at-home programs are experiencing growth and success. Top programs at Atrium Health, Mass General Brigham, and Mayo Clinic have reported significant impacts. Atrium Health has served 8,000 patients, with plans to increase their average daily census to 200 by 2025. Mass General Brigham has treated over 11,000 patients, with lower mortality rates and costs compared to inpatient treatment. Mayo Clinic has treated about 2,800 patients since its launch in 2020, with quality metrics showing equivalent or better care than a physical hospital.

The Future of Home-Based Care

With the integration of advanced technologies like augmented reality (AR), the future of home-based care looks even brighter. Dr. David Levine and Safwan Sarker from the Massachusetts General Brigham Home Hospital program presented their research on the integration of AR into home-based care. The study showed the feasibility of integrating AR technology with individuals receiving care at home, highlighting the potential for scaling the home care model. This development sparked conversations about the future of AR technology for home care patients and the impact of digital technology on the delivery and cost of care.

Challenges and Opportunities

Despite the promising potential of home hospital care programs, challenges remain. Identifying eligible patients and reimbursement issues for home hospital programs are among the top concerns. However, with continuous research, technological advancements, and commitment to patient-centered care, the future of home hospital care is poised for growth and success. By transforming the traditional hospital visit into a more personalized, comfortable, and effective experience, these programs are leading the way towards a more patient-centric healthcare future.
